New Account Request - Please download and complete the Academic Institution Form. Once completed, return to Joint Services Transcript via Academic Inquiry. You must verify your email before you can attach the form.

If you took the GED test(s) prior to September 1974, USAFI/GED transcripts are available for:
- U.S. military personnel tested before September 1974
- Ft. Jackson personnel tested before June 30, 1974
- U.S. Coast Guard personnel tested before January 1975
